Individual Residential Services and Supports – Host Home Placement and Independent Living

Life House Services, LLC (LHS) offers residential services to individuals of all levels of need to find a residential environment best suited for each individual to help them become not only a part of a family but to help increase their self-esteem and independence. Our services include a thorough matching process between clients and host home providers so that individuals are receiving the assistance they need to be comfortable, medically and physically stable, emotionally supported, and successful in everyday life tasks.

LHS understands that the host home is a large and an incredibly important element in the quality of an individual’s life. LHS is diligent in their monitoring to ensure not only the health and safety of each individual, but to ensure that individuals have access to their interests, their community, and all the services needed to be content, happy, and always moving forward.

Supported Community Connections

LHS offers services to individuals of all levels of need to access their community in a way that increases their self-confidence as well as their independence. We choose activities that increase life skills, social skills, and independent living skills. The activities are based on personal interests that will be discovered and outlined on an ongoing basis. We help individuals to not only do the things they know they enjoy but will help them try new things to find new interests.

This particular program focuses on community interaction through volunteering, educational trips, recreational classes, and community classes. These classes and activities involve anything from sports, cooking, crafts, music, photography, volunteer opportunities, and many more options that are available in the local community. This will not only allow them an opportunity to do or find something they love but also connect with other people in the classes or volunteer location who have similar interests. We encourage socialization in these areas. These activities will also help bring out natural talents in each person and help increase their participation and social skills as well.

Our staff will help find areas of interest, find community events, activities, and events that encompass these interests and transport and accompany individuals to each activity. The staff also helps individuals pursue new interests, activities, and skills.

Community Connection

A Life House Services Community Connector will offer one-on-one services to individuals of all levels of need to access their community in a way that increases their self-confidence along with their independence. We choose activities that increase life skills, social skills, talents, and independent living skills. The activities will be based on individual interests that will be discovered and outlined at the beginning of their enrollment, as well as through regular interest meetings and planning. We will help them not only do the things they know they enjoy but will help them try new things to find new interests.

The Community Connector will focus on helping individuals interact with their community through social events, volunteering, educational trips, recreational classes, and community classes. These classes and activities will involve anything from sports, cooking, crafts, music, photography, volunteering, and many more options that are available in their local community. This will not only allow them an opportunity to do or find something they love but also connect with other people in the classes or volunteer location who have similar interests. We will encourage socialization in these areas and activities.

Our staff will help find areas of interest, find community events, activities, and events that encompass these interests and transport and accompany individuals to each activity. The staff will also help individuals pursue new interests, activities, skills, and relationship with others in a non-integrated setting. These services will be offered as one-on-one according to their individual plan.
